Coordination of Care

The best outcomes occur when your medical team is coordinated and transitions in your care happen smoothly. Seneca Medical Advisory can assist with the communication of your medical information to and between all of your providers, as well as the organization and any transitions of your care.

  • We can collaborate with your doctors and other caregivers in person or by phone, to discuss any new and pertinent medical information, changes in care, outstanding questions, and continued forward movement of the care plan.

  • We understand how stressful surgery, hospitalization, and the recuperation process can be for both the patient and his or her family. We coordinate pre and post-surgical and hospital care, which includes:

    • Liaising with your medical team during your hospital or surgical stay to confirm that your hospital providers discuss your case and a unified plan has been established

    • Overseeing the implementation of any changes in care during the post-discharge transition to your home or rehabilitation facility, as well as assisting in the procurement of home health care services, such as physical therapy, a home health aide, and medical equipment

    • Communicating with your caregivers after surgery or hospitalization regarding your health status and well-being
